Only those who have registered their last name are allowed to permit a person of Thai nationality to use their last name.

Letter of Permission to share the last name (issued by the Registrar).
Procedures
The owner of the last name must file a request with the local Registrar where the House Registration Certificate is registered. The Last Name Registration Certificate must be presented upon filing.
The Local Registrar is required to review the documents for accuracy before issuing a Letter of Permission to share the last name.
A person filing for permission to share the last name is required to submit a request, including a Letter of Permission to share the last name, with the local Registrar where the House Registration Certificate is registered.
The Registrar shall review the documents before issuing a Letter of Permission to the applicant. A service fee of 50 baht is required.
After changing the forename and last name and sharing the last name, the applicant is required to submit the Letter of Permission issued by the Registrar to change items in the House Registration Certificate, and other related important documents, including a new Identification card.
